Observation

There were 2 childrens sleds in the back yard with "pull cords" that were capable of forming a loop and accessible to the children - provider removed during the visit. There is a rubber mat on the ground in the back yard play area that is lifting and is dangerous to the children. It is accessible to the children. The covered play area deck has flaking paint. There was a large cooler with hot containers that has soup in them. It was an entrampent and accessible to the children - the provider removed during the visit
